How Do You Know If You Need To Limit Gluten Or Fructans

Are You Gluten or FODMAP Sensitive?

Symptoms of IBS and coeliac disease can be identical, but managing them is very different. Knowing which approach is best for you is the key to getting maximum variety and minimal symptoms.

The first step is talking to your doctor and getting a proper diagnosis. Part of this process will be ruling out that you dont have coeliac disease.

Testing For Coeliac Disease

Testing for coeliac disease is a two-step process:

  • Blood test for coeliac antibodies tTg-IgA and DGP
  • If the blood test is positive, the next step is to have a gastroscopy that looks at the small intestine to see if there is damage caused by coeliac disease.
  • The tricky part of testing for coeliac disease is that both the blood test and the gastroscopy look for antibodies produced and damage caused when someone with coeliac disease eats gluten. So, if you are already on a gluten free diet, you can get a false negative.

    If you have already removed gluten from your diet, then you need to reintroduce gluten back into your diet to get an accurate coeliac disease testing result.

    However, if you have had a significant improvement on a gluten free diet, then the thought of doing a gluten challenge can be scary.

    Check The Ingredient List Well

    For gluten-free products that are not FODMAP-certified, it is important to read the ingredients list well. If a product only contains ingredients that are low FODMAP, then it is probably ok, even though the product hasnt been tested. If a product contains several ingredients that are high in FODMAPs, such as inulin or lupin flour, then you can better not take it.

    If a product only contains one ingredient that is high in FODMAPs in larger amounts, such as honey, but is totally on the end of the ingredient list, you can give it a try. It is probable that the amount of honey it contains is so low that it is still low FODMAP. One example of this is the Wholesome seeded loaf from Schär. The ingredients of this bread are:

    maize starch , water , sour dough 12% 16% , rice starch , cereals 4,3% 4,3% , vegetable fibre , sugar beet syrup , rice syrup ,sunflower oil , soya flakes 2,1% , sunflower seeds 2,1% , soya bran 1,9% , linseed 1,9% , thickener:hydroxypropyl methyl cellulose millet flakes 1,4% , soya protein , yeast , sea salt , honey . May contain traces of lupin . LACTOSE FREE .

    This bread has honey as the last ingredient on the ingredients list, but on their website it reads: This product has been certified to be low in FODMAPs under the Monash University Low FODMAP Certification Programme.

    What Is The Difference Between Coeliac Disease & Ibs

    Coeliac disease is a serious autoimmune condition where the body reacts abnormally to the gluten protein found in wheat, barley, rye and oats, resulting in damage to the lining of the small intestine. This damage interferes with the absorption of nutrients resulting in digestive upset, iron deficiency, osteoporosis, fertility problems and if left untreated, even lymphoma. Currently, the only treatment for coeliac disease is a strict gluten free diet.

    IBS affects approx. 20% of people globally and is far more common than Coeliac Disease. Up to 90% of people with IBS know that food is involved with triggering their symptoms and will modify their diet to help find relief. Although IBS does cause digestive symptoms, it does not cause damage to the intestine nor medical complications. Managing IBS is all about identifying the foods that trigger your symptoms and where your threshold for them is.

    Once you know how much or how little of those foods you can eat, you can then find the right balance between managing your symptoms and what food you can enjoy. Its okay if this balance is different on different days.

    Scenario #: You Can Eat Small Servings Of Bread Made With Regular Wheat Flour

    Researchers at Monash University created the FODMAP Diet, and have the done the vast majority of food testing. According to their app, a one-ounce serving of white bread is low-FODMAP. If you buy a typical loaf at a supermarket, one slice is usually about one ounce, or 28 grams.

    If you dont own the Monash FODMAP app, I highly recommend it–its the absolute best tool to understand the importance of portion sizes so can eat a wider variety of foods!

    If you eat a one-ounce serving in a sitting, you’re still in the low FODMAP zone. So, if you just love to have your regular piece of toast in the morning, you should be able to do that. One ounce is also the equivalent of half an english muffin. If you’re in a situation where you don’t have a lot of low-FODMAP food options, it’s nice to know that eating one piece of white bread likely doesnt contain enough FODMAPs to cause symptoms. Theres more flexibility in the FODMAP Diet than you might think!

    Let’s follow this logic a little further.

    If it’s okay to have a slice of bread, then it’s okay to have a few crackers, or a cookie that contains regular wheat flour. That’s not going to ruin your diet in the elimination phase, or trigger symptoms for most people.

    The goal is to reduce your FODMAP intake, not completely eliminate all FODMAPs . Truly eliminating FODMAPs would be nearly impossible, since many foods allowed in the elimination phase do contain some amount of FODMAPs.

    I like using coconut oil for this dish and I like using it in two forms and two ways. In its soft, but solid, form it works beautifully to make the streusel. For the batter, I like it in its liquid form, which is easy enough to make by zapping solid coconut oil in the microwave briefly until liquefied.

    Should I Be Tested For Celiac Disease

    If you think you have celiac disease, talk to your doctor before you make any dietary changes! To be correctly tested for celiac disease, you need to eat gluten for several weeks leading up to your test, or you may get a false negative.

    As we discussed earlier, celiac disease can cause physical damage to your body, even in small doses. So if you or your doctor think celiac disease may be the culprit behind your symptoms, complete these tests before you do anything else.

    Who Should Follow A Gluten

    People that have tested positive for celiac disease are recommended to follow a gluten-free diet. Celiac disease is an autoimmune reaction to the gluten protein, causing small-bowel mucosal lesion.

    Although celiac disease and IBS have some symptoms in common, celiac disease its a more serious condition, as it causes damage to the small intestine.

    In addition to celiac disease, there is a condition called non-celiac gluten sensitivity , which can cause symptoms similar to celiac disease, but without the intestinal damage. Those who think they may suffer from celiac disease or gluten sensitivity should see a doctor before starting a gluten-free diet, as it will make it harder to determine if the person has celiac disease or NCGS when gluten has been avoided for a while.7

    What Are Some Hidden High Fodmap Ingredients In Gluten Free Foods

    There are a number of hidden high FODMAP ingredients that are found in gluten free foods. These include:Soy:Soybeans and soy products can be sources of oligosaccharides, especially GOS. However, not all soy products are high FODMAP. Firm tofu is a curd made from soybeans, and the FODMAPs leach into the surrounding liquid, so firm tofu is naturally low in FODMAPs. Note: silken tofu is high FODMAP.

    High FODMAP fruits:The main FODMAP culprits in fruit are sorbitol and fructose. Fruits particularly high in FODMAPs are apples, pears, cherries, figs, pears, watermelon and dried fruit.Products with garlic and onion:This unfortunately catches many otherwise low FODMAP gluten free products out. It is important to get into the habit of looking for garlic and onion in the ingredients list of products. Common culprits are pasta sauces, chutneys, packet soups.Seeds/nuts:The main FODMAPs present in nuts are GOS and/or fructans. High FODMAP nuts include cashews, almonds and pistachios.

    Is the low FODMAP Diet Gluten Free?

    Dietary changes can have a big impact on IBS and SIBO symptoms, but doctors often use other therapies as well. Antibiotics can quickly reduce small intestinal bacterial overgrowth, while laxatives and low-dose antidepressants can relieve symptoms of irritable bowel syndrome.

    Which High Fodmap Ingredients Are Often Found In Gluten

    There are several flours that are gluten-free, but not low FODMAP. For example: soy flour, coconut flour, lupin flour and almond flour . For a complete list, you can check this blog post where I explain which flours are low FODMAP.

    Furthermore, high FODMAP fibers are often added to gluten-free products. Such as inulin, chicory fiber, oligofructose and fructo-oligosaccharides. Apple fibre has been tested and was found to be low FODMAP.

    Next to that, you can find juice of high FODMAP fruit- and vegetables in gluten-free products, such as apple juice, pear juice, beetroot juice or dried fruit . Sweeteners such as agave syrup, honey, fructose and polyols, such as sorbitol , mannitol , maltitol , xylitol , erythritol and isomalt are often added to sweeten products and these are high in FODMAPs.

    How Does The Low Fodmap Diet Work

    Low FODMAP is a three-step elimination diet:

  • First, you stop eating certain foods .
  • Next, you slowly reintroduce them to see which ones are troublesome.
  • Once you identify the foods that cause symptoms, you can avoid or limit them while enjoying everything else worry-free.
  • We recommend following the elimination portion of the diet for only two to six weeks, says Veloso. This reduces your symptoms and if you have SIBO, it can help decrease abnormally high levels of intestinal bacteria. Then every three days, you can add a high FODMAP food back into your diet, one at a time, to see if it causes any symptoms. If a particular high FODMAP food causes symptoms, then avoid this long term.

    Microbiota Analysis By Quantitative Real

    In the current study, a qPCR assay was performed for the enumeration of eight bacterial phyla, families, and genera including Firmicutes, Bacteroidetes, Actinobacteria, Enterobacteriaceae, Bifidobacterium, Lactobacillus, Ruminococcus, and Streptococcus. The qPCR was carried out by SYBR Green chemistry using universal and group-specific primers based on the bacterial 16 S rRNA sequences presented in Additional file : Table S2. Each PCR reaction was performed in a final volume of 25 µL, comprising of 12.5 µL of SYBR green PCR master mix , 1 µL of 10 pmol of forward and reverse primers, and 100 ng of the DNA template. The reaction parameters for amplification were 95 °C for 10 min and 40 cycles at 95 °C for 20 s, 30 s of annealing at optimal temperature for each primer pair as indicated in Additional file : Table S2, and 72 °C for 20 s. All PCR amplifications were carried out in triplicate by using a Rotor-Gene® Q real-time PCR system. The accuracy of amplification was determined by melting curve analysis with increasing temperature from 60 to 95 °C to confirm the specificity of amplification. The relative abundance of each taxon before and after the dietary therapy was calculated according to the ratio of 16 S rRNA copy number of specific bacteria to total 16 S rRNA copy number of universal bacteria using the previously described method . Accordingly, the average Ct value obtained from each primer pair was transformed into a percentage using the following formula:

    Who Should Try It

    The low FODMAP diet is part of the therapy for those with IBS and SIBO. Research has found that it reduces symptoms in up to 86% of people.

    Because the diet can be challenging during the first, most restrictive phase, its important to work with a doctor or dietitian, who can ensure youre following the diet correctly which is crucial to success and maintaining proper nutrition.

    Anyone who is underweight shouldnt try this on their own, says Veloso. The low FODMAP diet isnt meant for weight loss, but you can lose weight on it because it eliminates so many foods. For someone at an already too low weight, losing more can be dangerous.

    Does A Low Fodmap Diet Benefit Someone With Celiac Disease

    While many patients with Celiac disease will experience symptom improvements while following a strict gluten-free diet, a portion will continue to experience GI symptoms. This can likely be attributed to the fact that Celiac disease and IBS share a wide range of symptoms, including abdominal discomfort and bloating. People newly diagnosed with Celiac should work closely alongside a dietician to ensure that small amounts of gluten arent sneaking into their gluten-free diet. The FODMAP diet has gained traction over the last 5 years, with benefits becoming more and more clear as research progresses. Studies have shown that a low FODMAP diet can be beneficial to those with Celiac disease who experience IBS-like symptoms . It is worth noting that continuing symptoms may also be associated with factors unrelated to food, such as stress or emotions.
