Yes. The symptoms of dermatitis herpetiformis can come and go. There are periods of remission and periods of flare. Remission is spontaneous, and only about 12% of people experience it. Even if youre in remission it is still recommended that you eat a gluten-free diet.

Celiac disease is very specific. It is a genetic autoimmune disorder, in which eating gluten causes white blood cells to attack the lining of the small intestine. Over time, this causes poor absorption of nutrients from food, and triggers well over 275 associated symptoms. Celiac disease is diagnosed with a blood screening, and sometimes with a biopsy of the small intestine. The only solution is maintenance of a gluten-free diet for life.

Wheat allergy is a simple immune response to any of the proteins found in wheat. Absolutely not. Remember, DH is another form of Celiac disease. The only way to effectively treat Celiac disease is only eating a gluten-free diet.

In Celiac disease, a gluten exposure is likely to cause symptoms for 1-2 days. In DH, a single gluten exposure can cause symptoms for weeks. If youre just starting a gluten-free diet, be patient. You may not notice any change in symptoms for months. To further complicate the issue, your skin may continue to react for more than two years after starting a gluten-free diet. This makes it possible to get flare-ups or outbreaks even without a recent exposure to gluten.

Most individuals get diagnosed with DH in their 30s or 40s. That means the average person has likely consumed wheat products for more than three decades. Be patient it can take a year or more for symptoms to improve. Even though you may still be suffering symptoms, stick to a gluten-free diet for the long term. Eating gluten free is the only cure for DH.

But he gave me very little guidance, so I started reading books and labels.

Brown eventually took a blood test for coeliac but tested negative because she had already been gluten-free for a year.* However, an endoscopy confirmed she had coeliac disease.

Suzanne Beach, 55, of Bellefontaine, Ohio, had stomach problems before she was diagnosed with celiac disease and DH. Sometimes these were so severe she would end up in pain on the bathroom floor. She lost 26 pounds and was hospitalized before doctors figured out what was wrong. But still, what really upset her was the bilateral rash typical of DH on her knees and elbows.

Number one, its really itchy, way beyond having poison ivy, Beach says. Shes grateful that she has only had two instances of the rash since being diagnosed. Number two, Im out in the community a lot and having the visible rash would drive me crazy, she says.

Tania Fleming, 50, of Springfield, Pennsylvania, developed the rash nine years ago. Then, while taking a long road trip vacation, the itching became unbearable. It was extremely uncomfortable.

Cathy Stevenson, 60, of Saint Lawrence, Pennsylvania, sums up her symptoms prior to the gluten-free diet this way: It itches insanely. Her description is no exaggeration. In fact, originally the rash was thought to be a psychiatric condition because of an association with suicide.

For some people, celiac disease causes an itchy, blistering rash known as dermatitis herpetiformis. It may begin with an intense burning sensation around the elbows, knees, scalp, buttocks, and back. Clusters of red, itchy bumps form and then scab over. It often first occurs in the teenage years and is more common among men than women. The rash usually clears with a gluten-free diet but can also be treated with medication.

Gluten Sensitivity Without Knowing

Celiac disease and wheat allergy are quite unusual. While the symptoms of celiac disease are quite clear, the ones for gluten sensitivity are more blurry. Therefore, they can affect many people who are not aware that they have this problem, since their symptoms are similar to the ones from other diseases.

How Is Celiac Disease Rash Treated

Unfortunately, DH is stubborn. “Patients need to be on a strict gluten-free diet for two years,” Dr. Alesci says. It can take that long for the rash to begin to clear.

Going gluten-free eases symptoms a lot quicker for people who have celiac disease without DH. “When patients take gluten out of their diet, they feel better within days to weeks,” Dr. Burkhart explains. But with DH, the duration of the rash-even with treatment-is brutal because of the itchiness.

That’s why a gluten-free diet isn’t the only treatment for DH. You’ll also need relief from the itchiness and burning of the rash in the meantime.

Dermatologists are experts in the skin and are best for helping diagnose and treat DH. If you’re diagnosed, your provider will prescribe dapsone, an oral antibiotic and anti-inflammatory that will help relieve the symptoms of the rash. Sometimes, dapsone may need to be taken for one or two years, according to Dr. Alesci.

If dapsone doesn’t help much, then patients might also use a topical hydrocortisone or lidocaine cream for relief, Dr. Burkhart says. Dapsone can also cause side effects, includinghemolytic anemia, which is when red blood cells are destroyed faster than they can be made. In these cases, alternative medications including sulfapyridine or sulfasalazine may be prescribed.
